Incident Investigation

A dangerous incident is an incident in relation to a workplace that exposes a worker or any other person to a serious risk to a person’s health or safety emanating from an immediate or imminent exposure to: an uncontrolled escape, spillage or leakage of a substance.

When incidents are investigated, the emphasis should be concentrated on finding the root cause of the incident so you can prevent the event from happening again. The purpose is to find facts that can lead to corrective actions, not to find fault. Always look for deeper causes.

Section: 18 Functions of the Health and Safety Representative

One of the Functions of the Safety Rep is to conduct investigation of any incident or accident that occur in the workplace
